Working with the NQP and Nassau University Medical Center you will educate and support patients and/or their families affected by diabetes, help them understand and manage the condition, and promote self-management of diabetes to achieve individualized behavioral and treatment goals that optimize health outcomes. Bilingual English/Spanish required.
The DSRIP Project Manager of Quality supports the Quality Management and Performance Improvement activities on behalf of the Nassau Queens Performing Provider System (NQP). Working with the Director of Long Term Care, they will manage complex, interdisciplinary projects related to Skilled Nursing Facilities (SNFs) which is intended to improve performance.
In this role, you will be responsible for supporting the Project Managers by coordinating the day-to-day project activities and operations of one or more DSRIP projects to improve health care quality for the Medicaid population as outlined in NYS DSRIP.
